If you are outside the network, a Calendar Year Deductible of $500 per Insured Person must be satisfied before benefits will be paid.

A Stop Loss of 10,000 per person applies outside of the network. If you receive care within the network, we pay benefits at 100% and the Stop Loss does not apply.

* Maternity Coverage as long as conception occurs after the effective date of coverage or if you had prior group coverage.
